Submission of this form provides an official/binding record of the commitment of the athlete noted below to accept or decline the offer to play for Advancement Academy or A2 East during the 2019-2020 Club Volleyball season.

Choosing to accept this offer means you that you plan to play for adidas Advancement Academy or A2 East for the upcoming 2019-2020 season and you cannot tryout or play for another club. Please note: The club deposit of $500 is non-refundable and this deposit is required to secure your official acceptance of this offer.
Since you are accepting this offer to play with Advancement Academy, please be sure to go to USAV Webpoint, purchase a full season membership, and be sure to select "adidas A2" as your club.

Payment Instructions
If you selected "Payment via Paypal", upon clicking the Submit button, you will be sent to PayPal to complete the transaction. PayPal is a leader in online transaction processing and ensures your transaction is handled securely. Note that Advancement Academy does NOT see your credit card information since the transaction is handled entirely by PayPal. Once you complete your payment through PayPal, payment notification will then come directly from PayPal and serve as your receipt. If you cancel your transaction upon reaching PayPal, you will not receive credit for payment and you must pay via check or through our Club Payment tab on the website.
